5. Board review questionnaire

It is useful for boards to complete a Board Review questionnaire on an annual basis. The questionnaire can be completed anonymously and the collated responses tabled for discussion.

If the board review questionnaire systematically works through board accountabilities, board processes and the experiences of individual Board members in being on the Board reviewing the collated responses will provide an opportunity to systematically review the work of the Board.

One way of enabling Board members to anonymously complete Board Review questions and automatically collate the responses is through an online survey.

Survey B Board Review below is an example of an on-line survey that board members could use to review their work.

Surveys A, C and D gather other organisational review information. All four surveys can also be used as paper surveys.

A. Organisational Review - To what extent does your organisation have the characteristics of a thriving organisation? This could be completed by managers, staff or other people who are very familiar with the organisation.

B. Board Review< - To what extent is your Board working well and governing well? This is for board members to review their own work .

C. Staff experience of work - How do your staff experience working for your organisation? This is for staff to complete.

D. Stakeholder views of your organisation- How do other services and stakeholders perceive you? This is for other other stakeholders and services to complete.
